Advance Directive (ME)

This interactive questionnaire helps you create a Maine Advance Health Care Directive - a legal document that protects your right to refuse medical treatment you do not want, or to request treatment you do want, in the event you lose the ability to make decisions yourself.

It has 4 parts. You must complete either Part 1, Part 2, or both. Parts 3 and 4 are both optional.

Part 1: Power of Attorney for Health Care, lets you name an individual as an agent to make health care decisions for you if you do not have the capacity to make your own decisions or if you still have the capacity to make those decisions but want someone else to decide for you.

Part 2: Instructions for Health Care, in Maine's living will. It lets you express your wishes for receiving, withholding, or withdrawing treatment to keep you alive, including artificial nutrition and hydration, as well as pain relief medications.

Part 3: Donations of Organs at Death, is an optional section to record your wishes for organ, tissue, and body part donations after your death.

Part 4: Primary Physician, is an optional section to designate a primary physician.
