This document set consists of a Health Care Directive and a Living Will.

The Health Care Directive has two parts. Part A is the Appointment of Health Care Agent. Part B is the Advance Medical Directive Health Care Instructions, whereby you can direct whether you want life-sustaining procedures if you become terminally ill, enter into a persistent vegetative state, or enter into an end-stage condition.

The Living Will allows you to make decisions about life-sustaining procedures if, in the future, your death from a terminal condition is imminent despite the application of life-sustaining procedures or you are in a persistent vegetative state.
