Advance Directive (NM)

This document allows you to protect your right to refuse medical treatment you do not want, or to request treatment you do want, if you lose the ability to make decisions yourself. Specifically, you can:

  1. Name another individual as agent to make health care decisions for you if you become incapable of making your own decisions or if you want someone else to make those decisions for you now even though you're still capable.
  2. State your wishes for medical care in case you can no longer speak for yourself and are terminally ill or permanently unconscious.
  3. Designate your primary care practitioner.
