Advance Directive (OR)

This questionnaire will create an Advance Directive that meets the requirements of Oregon law. You have the option for your Advance Directive to include Health Care Instructions or an Appointment of Health Care Representative, or both. Health Care Instructions (also known as a Living Will) describe the care you wish to be provided in the event of your incapacity. An Appointment of Health Care Representative (also known as a Health Care Power of Attorney) is used to name someone to make decisions about life-sustaining treatment if you are in a terminal condition and can no longer speak for yourself.
