Living Will and Health Care Power of Attorney (AZ)

This questionnaire helps you create a Durable Power of Attorney for Health Care and/or a Living Will for a resident of Arizona.

1. Durable Power of Attorney for Health Care: This lets you name another person to make decisions about your medical care, including decisions about life support, if you can no longer speak for yourself. You can also include preferences for voluntary autopsy, organ donation, and what happens to your remains.
2. Living Will: This lets you state your wishes about medical treatment, including end-of-life treatment, if you develop an irreversible condition that prevents you from making your own medical decisions.
