Living Will and Health Care Power of Attorney (AR)

These documents allow you to appoint a health care agent to make health care decisions for you when you become unable, and express your wishes for life-prolonging care in an end-of-life situation. Specifically, these documents are:

  1. Durable Power of Attorney for Health Care, which lets you designate a person as your health care agent to make health care decisions for you when you become incapable of doing so yourself.
  2. Declaration (Living Will), which lets you state your individual instructions for health care. It lets you express your wish regarding withholding or withdrawal of life-sustaining treatment when you are unable to make decisions regarding medical treatment.
  3. Anatomical Gift, which lets you express your wishes for donation of organs, tissues, and other body parts, which could be used for therapy, medical research, and education.
