Living Will and Health Care Power of Attorney (AR)
These documents allow you to appoint a health care agent to make health care decisions for you when you become unable, and express your wishes for life-prolonging care in an end-of-life situation. Specifically, these documents are:
- Durable Power of Attorney for Health Care, which lets you designate a person as your health care agent to make health care decisions for you when you become incapable of doing so yourself.
- Declaration (Living Will), which lets you state your individual instructions for health care. It lets you express your wish regarding withholding or withdrawal of life-sustaining treatment when you are unable to make decisions regarding medical treatment.
- Anatomical Gift, which lets you express your wishes for donation of organs, tissues, and other body parts, which could be used for therapy, medical research, and education.
