Living Will and Health Care Power of Attorney (DC)

The District of Columbia Declaration (Living Will) and Durable Power of Attorney for Health Care is a legal document that allows you to express your wishes regarding the use of life-sustaining treatment when you become unable to do so and appoint an agent to make health care decisions for you.

  1. Durable Power of Attorney for Health Care - Lets you designate a person as your agent to make health care decisions for you when you become incapable.
  2. Declaration (Living Will) - Lets you state your individual instructions for health care. It lets you express your wishes regarding the withholding or withdrawal of life-sustaining treatment when you are unable to make decisions regarding medical treatment.
  3. Anatomical Gift - Lets you express your desires about the donation of organs, tissues, and other body parts, which could be used for therapy, medical research and/or education.
