Living Will and Health Care Power of Attorney (NY)

This interactive legal questionnaire enables you to generate either a Health Care Proxy, Living Will, or both.

The Health Care Proxy allows you to name someone to make decisions about your medical care, including decisions about life support, if you can no longer speak for yourself.

The Living Will allows you to state your wishes about medical treatment in the event that you develop an irreversible condition that prevents you from making your own medical decisions.

(Note: Unlike most states, New York does not have a specific law recognizing living wills. However, you may complete a living will to more clearly communicate your wishes about care in case you become terminally ill, permanently unconscious, or minimally conscious due to brain damage and will never regain the ability to make decisions.)
