This questionnaire helps you create legal documents that allows you to appoint a health care agent to make health care decisions for you when you become unable, and express your wish regarding the use of life-prolonging procedures in an end-of-life situation. Specifically, these documents are:

  1. Health Care Power of Attorney, which lets you designate a person as your health care agent to make health care decisions for you when you become incapable of doing so yourself.
  2. Advance Directive for a Natural Death ("Living Will"), which lets you state your individual instructions for end-of-life care such as withholding or withdrawing life-sustaining treatment if you are terminally ill.
