Living Will and Health Care Power of Attorney (PA)

This questionnaire will create an advance directive, a single document containing several parts including a Durable Health Care Power of Attorney and a Health Care Treatment Instructions (Living Will). You can choose to complete one or both of these parts. A Health Care Power of Attorney authorizes a health care agent to make decisions for you (the principal). The Health Care Treatment Instructions (Living Will) helps you express your wishes regarding initiation, continuation, withholding or withdrawal of life sustaining treatments and may include other specific directions.
