Care Agreement (Personal and Elder Care) (NE)

This is a Care Agreement to be used when hiring a caregiver for a person requiring personal care assistance, to help with medical care, cooking, housekeeping and other needs. The document will outline the caregiver's responsibilities, hours, benefits, amount of pay, schedule of payments and other important details.

If you are using this for the care of a person other than yourself, you should have power of attorney or some other authority recognized under state law.
