Advance Directive (MI)

This packet contains two legal documents that protect your right to refuse medical treatment you do not want, or to request treatment you do want, in the event you lose the ability to make decisions yourself: 1) The Michigan Designation of Patient Advocate for Health Care lets you name someone to make decisions about your medical care if you can no longer speak for yourself; and 2) The Living Will, which allows you to state your wishes about medical care in the event that you become terminally ill, permanently unconscious or minimally conscious due to brain damage and permanently unable to make decisions or express your wishes.
