Advance Directive (NH)

The New Hampshire Advance Directive is a legal document that protects your right to refuse medical treatment you do not want, or to request treatment you do want, in the event that you lose the ability to make decisions yourself. Part 1, Durable Power of Attorney for Health Care, lets you name another individual as agent to make health-care decisions for you if you become incapable of making your own decisions. Part 2, Living Will, lets you state your wishes about medical care in the event that you can no longer speak for yourself and you are terminally ill or permanently unconscious. Choices are provided for you to express your wishes regarding the provision of artificial nutrition and hydration.
