This document allows you to appoint a health care proxy to make health care decisions for you and express your wishes for life-prolonging care in an end-of-life situation. Specifically, you can:

  1. Name a health care proxy and alternate health care proxy to make health care decisions for you.
  2. State your instructions for end-of-life care such as withholding or withdrawing life-sustaining treatment if you are terminally ill.
  3. Select an option for organ or tissue donation, including the option not to donate.
