Advance Directive (TX)
This interactive questionnaire helps you create a Texas Advance Directive - a legal document that protects your right to refuse medical treatment you do not want, or to request treatment you do want, in the event you lose the ability to make decisions yourself.
It has two parts, and you may complete one or both of them:
- Medical Power of Attorney - Allows you to name someone to make decisions about your medical care, including decisions about life support, if you can no longer speak for yourself.
- Directive to Physicians and Family or Surrogate - Allows you to instruct doctors about which life-sustaining treatment you would or would not like to have if you are in an irreversible or terminal condition and unable to communicate.
