Advance Directive (WY)
This questionnaire lets you create an Advance Health Care Directive which protects your right to request treatment you want and refuse medical treatment you do not want if you lose the ability to make decisions yourself. The advance health care directive has four parts:
Part I. Power of Attorney for Health Care. Allows you to appoint a person to take health care decisions when you become unable to do so.
Part II. Instructions for Health Care (Living Will). Allows you to give directions regarding the use of life-prolonging medical intervention when you become unable to take your own medical decisions.
Part III. Organ Donation. Allows you to express your desires about donation of organs, tissues, and other body parts, which could be used for therapy, medical research, and education.
Part IV. Information about Primary Physician. Allows you to designate a physician to act as your primary physician, who will be responsible for your health care.
