Living Will and Health Care Power of Attorney (LA)

This questionnaire helps you create a Declaration concerning life-sustaining procedures (Living Will), and/or a Durable Power of Attorney for Health Care, for a resident of Louisiana.

1. Declaration (Living Will): This is your personal set of instructions for health care. It lets you give directions about the use of life-sustaining procedures when you become unable to make your own medical decisions.
2. Durable Power of Attorney for Health Care: This lets you appoint a person to make health care decisions when you become unable to do so.
