Health Care Power of Attorney (AZ)

This durable power of attorney for health care lets you name another person, called an "agent," to make decisions about your medical care, including decisions about life support, if you can no longer speak for yourself.

You can place limits on their authority and as well as appoint an alternate agent in case your first agent is unavailable. There are also options to include preferences for voluntary autopsy, organ donation, and what happens to your remains.
