Living Will and Health Care Power of Attorney (IA)
These documents allow you to appoint a health care agent to make health care decisions for you when you become unable and express your wishes for life-prolonging care in an end-of-life situation. Specifically, these documents are:
- Declaration Relating to Life-Sustaining Procedures (Living Will), which states that you want life-sustaining procedures to be withheld or withdrawn if you are unable to make treatment decisions, and you have an incurable or irreversible condition that will result in death in a short time, or are permanently unconscious. You may also include additional end-of-life care instructions.
- Durable Power of Attorney for Health Care, which lets you designate a person as your health care agent to make health care decisions for you when you become incapable of doing so yourself.
- Anatomical Gift, which lets you to share your wishes for donating organs, tissues, and other body parts, which could be used for transplant, therapy, medical research, and/or education.
