Living Will and Health Care Power of Attorney (NE)
These documents allow you to appoint an attorney in fact to make health care decisions for you when you become unable, and express your wishes for life-prolonging care in an end-of-life situation. Specifically, these documents are:
- Power of Attorney for Health Care, which lets you designate a person as attorney in fact to make health care decisions for you when you become incapable.
- Declaration (Living Will), which lets you state your personal instructions for health care. Specifically, it lets you express your wish to withhold or withdraw life-sustaining treatment in an end-of-life situation when you are unable to make decisions about medical treatment.
- Anatomical Gift, which lets you express your wishes for donation of organs, tissues, and other body parts, which could be used for therapy, medical research, and education.
