Durable Health Care Power of Attorney and Treatment Instructions (Living Will) (PA)
This document allows you to appoint an agent to make health care decisions for you, and express your wishes for life-prolonging care in an end-of-life situation. Specifically, you can:
- Name an agent and alternate agent(s) to make health care decisions for you
- State your individual instructions for end-of-life care such as withholding or withdrawing life-sustaining treatment if you are terminally ill
- Select an option for organ or tissue donation, including the option not to donate
You can choose to complete some or all of these sections. It's recommended to complete all sections to best ensure you receive the medical care you want when you can no longer speak for yourself.
