This document lets you appoint an agent to make health care decisions for you when you are incapacitated and no longer able to make those decisions for yourself, and lets you express your wishes for life-prolonging care in an end-of-life situation. Specifically, you can:

  1. Name an agent and alternate agent to make health care decisions for you
  2. Indicate your wishes for quality of life
  3. Indicate your wishes for treatment if your quality of life becomes unacceptable to you
  4. Select an option for organ or tissue donation, including the option not to donate
