Request a Website to Supply all Personally Identifiable Information (MD)

Depending on your state, you may have a legal right to request a copy of your personal data from various websites. Even if you do not have a legal right to the information, most companies will provide you with information regarding any of your personal data that they have stored or shared. This letter may be used to request access to any data a company may hold regarding you or someone you represent under a Power of Attorney or court order. The letter will allow you to enter identifying information for yourself or the data subject to help the company locate your information. It will also allow you to specify the type of information and uses of such information. Keep in mind that each company may have a unique process for making these types of requests so be sure to check their website. Additionally, if you are representing another individual, you will likely be required to provide proof that you have authority to act on that person's behalf. This may require you to submit a copy of the authorizing court order or Power of Attorney document along with your request.
